New Patient Registration

The doctors welcome new patients who live within our practice area (MK16 and Redhouse Park).  As it often takes sometime for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Please see the attached list of doctors currently taking new patients.

On registering, all new patients will need to undergo a medication review with the new GP. Only those medications or treatments deemed necessary for the health of the patient and which comply to the latest pharmaceutical advice will continue to be prescribed/provided by doctors at this Practice.  Any treatments from other agencies from home or abroad will also need to be reviewed.

Medical treatment is available from the date of registration.

Pre-registration with the practice

When you visit the surgery for the first time you will be asked to complete and sign the pre-registration form.  Alternatively you may wish to print off the form, fill it out and bring it with you on your first visit to the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. You may wish to print off the form, fill it out and bring it with you on your first visit to the practice.

From 1 June 2011, when registering as a new patient we will need information to prove your identity and separate information to verify your address.  Please refer to the attached document which lists items that are considered as acceptable identification. 

A birth certificate is also required when registering anyone under the age of 18.

Please note we require the pre-registration form, new patient questionnaire and verification of identity and address to enable us to register you.
